A Continuum of Care (CoC) is a regional or local planning body that coordinates housing and services funding for homeless families and individuals. The goal of a CoC is to provide a structured, comprehensive approach to addressing homelessness by ensuring that those in need have access to a range of services, including emergency shelter, transitional housing, and permanent supportive housing. By fostering collaboration between service providers, government agencies, and community organizations, a CoC works to streamline resources and enhance the effectiveness of homelessness assistance programs.
